Well I mean that people in beginning stages of Alcoholism or Opiate addiction wouldn't be immediately obvious. They might only be taking it once a week, have a job, friends etc and it takes some time for tolerance and addiction to truly build up.

That's exactly what they think. It's a fight between short term and long term rewards that they're incapable of winning as a depressed person. To put yourself in the eyes of a superfatty, the ending should sound more like:

These people don't think in terms of what's good for them. It's how they feel. How would you feel if you woke up and that was what you looked like?
