In every 80s and early-mid-90s movie where the protagonist is a wimpy kid, or has a wimpy friend, who have to deal with a bully, the bully always does this move where he takes on hand, grabs a kid by the shirt at the neck, cocks his arm back slowly, says something like "You're dead meat!" and then punches his lights out.
It seems unavoidable as we're never shown a protagonist getting out of it without someone intervening. So how exactly do you defend yourself against this move (shirt grab punch)?

1) This doesn't happen in real life. Shirts get grabbed but to obscure your vision with your shirt. It can be also used to hold you in place but rarely by the front collar.
2) This puts you at an advantage. You are now fighting a person with one arm while you have two. You can use one arm to block and you punch them in the face with the other.
3) They are giving you their arm. Grab it fall to the ground and attempt an arm lock. If successful you can even break their arm.
Watch some street fights and join some karate classes. At least what some video on fighting technique.

You seriously can't telegraph an attack any more than that and telegraphing is a fucking big mistake. The only reason that works in movies is because the wimpy kid is a wimpy pussy who isn't used to fighting and doesn't have the faintest idea how to defend himself. He freezes rather than do anything, and is also outclassed af.
Oh and in my experience being punched isn't all that. Sure it hurts afterwards and if it's a good blow it might leave you stunned / a bit glassy-eyed but if you know what to do beforehand and actually react with purpose then that's all you need.
The wimpy kid always leaves it at that one punch after which the bully turns his back and wanders off. Instead you could charge an elbow into the back of his head with all your momentum behind it, put into a chokehold, find a weapon and smash him in the side of the head with it. Whatever.

I didn't suggest starting with a headbutt 'cause using the OPs pic as an example, the wimp probs doesn't have the strength to land a strong headbutt from that position. The bully is able to use his grip to somewhat control the wimps position. Whereas if the wimp puts his hand around the bullly's neck in a collar tie he's then able to use both arms and much more of the force of the rest of his body to easily overpower the force applied from the bully's one grabbing arm. From there, knees, headbutts or whatever willl be much more effective.

Put your leg between his and hook it behind one of his legs, (referred to as an inside trip) and just grab his face and direct it towards the ground in an arc like pattern. If not done on a padded surface and if the opponent doesn't have proper landing technique he will be seriously injured as the back of the skull is very soft and you will effectively be palm striking his face.
