1 line of backstory isn't usually enough to develop the patients issues, so I usually stick with 2. 3 works on occasion, but can make the joke to long and boring and make the reader uninterested because of the author's blabbing.
And yes, the penultimate line has to be

But the ultimate line can rarely vary from

Example
>robot walks into the surgery
>claims his depression has become too crippling
>tells me he has taken every antidepressant possible
>he wants to fall into an eternal sleep
>asks me for a lethal dose of morphine
>give him a non lethal dose of prozac instead
In this example I used both the optional third line and the reversed punchline. They work well depending on the poster's ability
