Quick rundown of what I do.
Fix spelling mistakes
Make the voice speak new terms properly (or as close as I can get.) Lemme tell you, Final Fantasy XII fanfiction was interesting...
Add pauses in dialogue, between sections, and paragraphs to make it more obvious when each is happening.
Lastly (and MOST importantly) I change pitch between Dialogue and Narration, so that it's like 20x times easier to tell the difference.
I'm working on ways to make italicized text obvious too, but have not yet uploaded a book with that feature.
