You have to separate Shkreli's personality from his actions. He's been very successful and he knows what he's doing. What happened to him has been fairly unprecedented. I work in pharma R&D and it was a riot to watch this.
First of all pyrimethamine's usefulness was distorted. It doesn't treat HIV/AIDs at all. It only treats toxoplasmosis is immune compromised people. In people with healthy immune systems toxoplasmosis is rare and if contracted doesn't require treatment generally.
Second, pyrimethamine was widely considered past its useful even for treating toxoplasmosis in later stage AIDs patients. Bactrim is far more useful for this and very cheap. Pyrimethamine was essentially a drug of last resort, it might be used when Bactrim failed, but in all honesty if you've got toxoplasmosis THAT rampant you're terminal. Your immune system is gone and it's only a roll of the dice what mixture of pathogens is going to kill you.
Because pyrimethamine was no longer useful it was essentially a very niche drug. Pharmacies wouldn't stock it at all. It was exclusive to hospitals as any patient THAT immune compromised would be hospitalized. Pyrimethamine had a very small and specific scope of use. High immune compromised individuals who had failed to respond to other antiprotozoal treatments and who didn't want to die.
These types of drugs are VERY expensive. Pharma companies have balance the cost of producing them with very limited market for consumption. Niche drugs like this are universally incredibly expensive.
Long story short the price hike wasn't unusual. Pharma companies do this all the time. The price of the drug after the hike wasn't unusual. The media took the application of the drug and price increase out of context, "look he's denying AIDs patients medicine". The drug didn't even treat HIV/AIDs and even then toxoplasmosis in these people is rare.
There was just a massive smear campaign to ruin the guy. I've no clue why.
