I'm cursed with very wide feet, with one a bit wider than the other, so finding good-fitting shoes is a huge pain in the ass. In fact, the reason I don't bother to dress well is because finding sneakers that don't look like ass in a 4E is nearly impossible. It's dress shoes only at that point.

Buy custom. Find a local cobbler that does decent work and ask. Seriously, a good custom pair of shoes will cost around $250 for a standard shoe, and up to $800 for detailing, odd materials, or simply finding a more pricey artisan. I suppose this is the different between custom and bespoke as well.
