When i'm washing my hands i just turn down my head avoiding looking at myself
I simply don't like how i look
I also use sunglasses everyday so no one can see my face

have you tried working on your appearance?
diet, orthodontics, working out, IPL photo-rejuvenation etc..you can actually work on your looks; although you need to put up with looking worse for a couple weeks if you do something like laser treatment

One beautiful thing about getting older, like past 26-28, is that you stop caring so much.

When you are not used to a person they look different, once you get used to a person you feel different towards them, in this case, OP is a stranger to himself, he may not be familiar with his image, if her were asked to describe himself to a sketch artist he would not be able to give an accurate description.
