Doctor literally stopped asking after I turned 20 and just assumes everyone is after that point.

Like to the point where my blood work got a STD screening by default.

If for some reason a health care profecional ask I would tell them the truth though because it is not a big deal to me. But it never comes up.

In other words I would just say no and wait for the next question without making a big deal about it.

There's usually no need to ask this during a consultation unless it is already specifically about STDs or fertility issues. There are sexually transmitted things that present with systemic signs but they are rare so no one bothers to ask initially.
