You do not have to worry about mental problems at all cause what it's more likely to do is ease such troubles. It's nowhere near self induced schizofrenia, especially considering it's impossible to induce oneself with it, or so I presume. But in no way can the tulpa concept mentally scar You. Anything else You need? Not really, although I do not really recall the guide well. However I'd say practice, meditation, anything that comes up in yout mind in regards to the subject would be of much help. Visualization, invokation, playing around the wonderland, experiment I'd say, that's one of the many beauties surrounding all of this, for each journey is very own and unique.
As for the appereance, I think it is useful to have some kind of idea or concept, suggestions are neat too. So You can give them a finished/polished/chiseled and ready look. While on the other hand You can either just gently guide them towards some style or idea, if not let them form on their own which is very interesting. The only drawback of leaving it completly to them might be lack or lesser amount of detail.
Therefore I would suggest what I personally prefer, being gentle guidance and nurturing their bloom. A lovely, well tought balance of autonomy and unpredictable change, partnership/co-operation. I don't know how to name it but I hope You get the overall idea.
Almost as with the golden mean haha. It's just about keeping balance and harmony, playing along in this wonderous dance of expressions.
