The only reason traps are transitioning is because modern medicine can allow them to do so. Historically, the vast majority of those with the same mindset and impulses of a modern pre-transition tranny would just deal with it. The most extreme would either become celibate, join a monastery (forcing them to contribute to society), castrate themselves, or some combination of those. It's not courage driving them to transition, it's gratification fulfillment.
