I'm in the opposite boat, where my jaw actually turned out fairly respectable, but I have this large, rounded nose that is seemingly out of nowhere. No one in my family has it. I'd be preparing for it since I was 13 with other orthodontic procedures (braces, wisdom teeth pulled, etc) and so it was prepared for me by my family. It would take at least a year or two of planning for anyone interested because there may be other shit they need to do beforehand. So speak to an orthodontist first and then ask about the surgery. Adults can get braces too, they just might be less effective. The first step would probably to get healthcare, find orthodontists through them, see what other things may need to happen, and then get under the knife.
It's a massive improvement, my speech got better, my confidence sky-rocketed and I'd recommend it to anyone.
