I've had work done. In my opinion, it has made a dramatic difference but no one else has noticed. I didn't tell my immediate family or anyone I know and they didn't comment on it, which was made me feel much better.
My mother has had a lot of work done as well, but she goes to shoddy doctors and is generally irresponsible. I need a few more things done but they are so rarely done with expertise that I doubt I will get them any time soon.

Consult with as many doctors as you can, ask to see pictures of their work during the appointment, ask them if they have done patients with skin/bone structure/etc like yours. Seriously just consult a lot of doctors. In the double digits. It'll cost an additional amount of money but it'll be worth it. They'll all suggest doing different things/procedures and you'll have to go home and do your own research.
Get some therapy beforehand. Get fit beforehand. Do literally everything in your power to make yourself look better before you resort to invasive surgery. Only resort to plastic surgery as a last resort. When you go through with it, make sure you're mentally prepared and have a good support system otherwise you will probably go crazy after looking in the mirror for the first time and seeing your post-surgery face
