Medically speaking, there are psychogenic erections and reflexive erections. Psychogenic ones come from sexual thoughts and come from the brain itself, whereas reflexive ones come from response to physical stimuli and go to and from the spinal cord around your lumber/sacral area.
You can get erections from naughty thoughts and you can get them from shit your body's sense receptors interprets as sexual contact. There are probably other causes too within these but those are the two general categories.
