Does it have a lid up front that closes when it pulls in its head? Than its a boxturtle.
You better take it to a vet, even if the initial wound isnt that bad, considering its going to get colder soon and those turtles hibernate/sleep during the winter. So if its already sick or injured it will not survive the winter sleep.
Take it with you and take it to the vet, then keep it at your place where it is warm for the the winter/until temperatures are higher
