Jonah Hill was really fat before he lost that weight and he lost it really quickly.
Even then, the skin on his face would tighten over the course of several years if he had stayed thin.
The skin shrinks. You get some bunched-up wrinkles here and there, but no big deal. Usually saggy skin is just more fat clinging to the skin and not the body, so it hangs. If they lose that fat then the extra skin is paper thin before skrinking.
The problem is stretch marks. Scars everywhere.
