It may also mean that 4chan can't guess the country of the poster, this happens with some proxies.

It usually happens because of retarded mods. For instance threads about Japan being moved from /a/ to /int/, but more often from /pol/ (note that even if /pol/ has flags, they all become Unknown anyway when moved to another board, this guy here >>587163 is wrong).
