I group them by what board they'd be most relevant to. So I have an /a/ folder, /v/ folder, /out/ folder, etc. The only folder I have that really needs organizing is my /a/ folder because it's by and large the largest one. But for /a/ I just organize the pictures by anime or trope (like Japanese Trucks or girls eating burgers). The other folders mostly just have their images all mixed together.
It's pretty cluttered by I know where everything is half the time.

- many directories for each show and show-related stuff, if something isn't important enough it gets thrown in the board directories
- reactions/
- misc/
- important images are also copied here
I just have one big folder.
I don't have a folder of reaction images. I google image search for any reaction image I might need.

First level is by board.
If my board folders don't have a lot of images in them because it's a board I don't go on very often (like /sp/ or /his/ or /pol/) then I don't bother sorting.
Bigger folders, like /a/ or /v/, are first sorted by IP (name of anime/game) and then sorted by character. I also have a "reaction" folder for /a/ and /v/ so that I keep my reaction images separate.
My /a/ reaction folder is sorted by things like smug, crying, annoyed, sip, and so on.

Have a folder called "work" (I make a lot of presentations), and a folder called "play" with everything else in it.
Other than that I keep my computer clean.

I insert tags in the file name. It's a lot quicker and more useful as a method of organization.
Most folders are pretty straight forward except for /b/ which has a few specific meme / pron folders.

just out of curiosity, what do the tags look like?
I have all my reaction images in one big folder. Otherwise, my anime and anime downloading methods, along with other /a/ stuff is all relegated to one folder essentially.
