Yeah, I've had several subdivisions for pic types but think I need to start sorting the subdivisions by emotions instead of just date saved under looser categories. I just never get around to it because damn, that could take several hours, hence probably a while before I actually net a return benefit in the time saved because I don't often have a very specific face in mind and tend to make do with whatever is nearer to the bottom of my pile.
Sometimes I'll spend like 5-10 minutes looking for image I had in mind, and then get distracted looking at all my old reaction faces pondering "some these are great, wonder why I never use them anymore" and know it's because they got buried up in the folder by newer saves.

Organize your reaction images by folders. Either by character/franchise, or by theme. I prefer the latter. For example, I have a folder where I keep all my images of confused girls with question marks around them and I attach images from that folder to posts where I'm asking questions.

It's gotten out of control
I had my images sorted on my old computer. When that died I said I was going to wait until I imported the old pictures to my new computer rather than create any new folders. Two years later I've got nearly 30k unsorted images. I pretty much have to search by filename when I want to upload something, and reaction images are out of the question.

all my files are named (and often in a way where the name acts like tags), but some days I decide to be clever with the name and then I can't find the file ever again without manually looking through every one

I stand corrected. I apologize. I saved that image literally over a decade ago and never thought it could still be there.
I have this same issue. I can generally find a reaction face without issue because despite having 669 of them unorganized in my main folder, it isn't too hard to find one which is relevant. I recommend with reaction faces to not decide in advance which one to use and instead scroll through until you find one which suits your purpose.
My main issue is with screencaps which I have over 1000 of, completely unorganized and which I often want a particular one of. I have tried to think of a way to sort them but the only two ways I can think of are by year (which, while neat, doesn't really help my problem) or by topic i.e. funny, informative, story, etc. however this still has issues if I am looking for a particular board or other particular sorting method. What I might do is give each image multiple properties and then use this to find them.
I also have an issue with my 'other' folder (mainly memes or random images from 4chan) which has over 700 images and which I have little idea how to sort. My best bet there is probably to create detailed folders for everything but it is going to be an absolute mess to sort.
Pic related is my main folder. Many of these folders also subdivide further such as other with 5 subdivisions and reaction faces with 3.

I have ~2300 images, vast majority of them are in a single folder with most of them named with the timestamp. Still, I can remember the position of almost every image in my folder, all I need to do is scroll a bit to refresh my memory.

I have a few thousand that I use regularly in a folder and they all have names and can recall them all from memory when I'm searching. I either name them based on emotion or if I can think of something witty or a name with guaranteed replies, I go with that instead
