30 years ago cops never shot first. They were trained professionals against untrained, panicked civilians, even if a civilian got a shot off it'd likely miss and you just did not adopt a first-strike mentality. But our police are now War On Terror-era ex-soldiers who were trained to shoot first.

2. People are not taught how to interact with police properly during stops. Somehow I got an aside of how to do it correctly. Never reach for anything around a police officer. Even if they ask for your registration or insurance, you tell them first it's in your glove compartment and ask if it's okay to get it. The few times I've been pulled over, the cops seemed to appreciate it. Normally they will ask if there is a weapon and I say no. One time they kind of laughed it off like I was a retard for asking. That was also the only time I got a ticket. We had a case recently where a man opened fire on a police car with a submachine gun.
Cops, seeing that his magazine was empty, got out of their car and verbally commanded the man to get down. Of course they pulled their weapons out, but they showed immense restraint.

The standard for use of lethal force by a police officer is not "were they armed" it's "would a reasonable officer in a similar scenario have reacted in the same way". The kid wanted the police to think he had a gun. He did everything he possibly could to convince them he was armed. Every single move made by the kid had to be analyzed and reanalyzed in a split second. Did the cop understand that the kid might actually be unarmed? Probably.
