I'll try to explain.
My mrs. is a dentist. And I asked her about this. Americans often say we have bad teeth.
Teeth are not supposed to be white. Enamel is the coating for teeth and it isn't white.
You can have healthy teeth and no fillings or missing teeth, but whitening your teeth causes decay. It causes problems. It's putting harmful chemicals into your mouth.
Gum disease is a bigger problem. It can kill.
Always brush your teeth at least twice per day preferably after meals. Use mouthwash, and eat well. Your teeth will be fine.

I think the stigma is moreso regarding misaligned/crooked teeth rather than color.
Here, it's common for most people to get braces if their teeth are uneven from a young age. I would even say people who never had braces or a retainer may be the minority, though I don't know the actual statistics.
I asked my friend who is studying abroad about this. I postulated that Brits care about dental health, not so much appearance. But she told me that the Brits she's met are rather self conscious if they have crooked teeth.
So, the mystery remains a mystery for us Americans.
