Typically what happens is the paramedic begins attempting to revive the patient (if there are no obvious signs of death) then when it's thought that the patient can't be revived the person in charge of the scene will contact their medical director, relay all the important information, then the director will tell them to stop CPR and pronounce the person dead.
As an EMT with a private company we do hospice transports and this happens quite frequently. The person is actively dying and being taken to a specific place to die be it home or a hospice house. Sometimes they die in the ambulance and we'll call our director. He tells us to attempt to obtain a set of vital signs and when we fail to find a pulse we pronounce their time of death.
