Free speech is listed as one of the things protected under the NK constitution. Quote:

What does that mean? Obviously the constitutional article is vague, so we instead can get our boundary hints by using the NK criminal code, noting things like, quote:

or electronic media that reflects decadent, carnal or foul contents or who performs such acts

So immediately you should note that the freedom to 'express' does not include to 'possess' or even to 'listen', as it often does in US law.
Speech also can conflict with a number of other criminal statutes in regards to harm against the state. You CAN propose differences, improvements, etc. And you can talk pretty freely - BUT their version of speech is different than the US.
While the US makes its low value speech: commercial, violent, lies. NK makes its low value speech that which otherwise does not help the goal-oriented philosophy of the people-state.

Malnutrition was ABSOLUTELY RAMPANT in the late 90's. The famine was extreme. You can see the results of it in those who lived through it: stunted growth, starved features, etc. It's better now, so the younger generation is coming up healthier, but it still nowhere near ideal. Meats are not super common, so protein is still lacking for them gains.
