Tests and training are only needed for public use. Public carrying of guns requires tests and training.
The health requirements are pretty lax for driving are only there to ensure that the driver is physically capable of operating the vehicle and doing so within the most minimal safety parameters. There is no level of mental health screening for vehicles. An equivilent level of testing for firearms would be to see if the person is capable of picking it up, aiming and pulling the trigger.
Insurance requirements only exist because of the rates of accidents and cost per incident is high enough that its better for the public good to require insurance. The rates of gun accidents/defense is so low its not necessary to require this insurance.
Renewals are for revenue/requlation of public use. Most concealed carry states have an annual renewal fee which is about as much as registration.
Inspections for safety are unnecessary because catasprophic failure resulting in injury is incredibly unlikely. Most states inspect mainly for emissions anyways and that is not needed for guns.

Well who is the militia is where we can start. According to 10 U.S. Code § 311, it is all able bodied males 17 to 45. Per the 14th Amendment, this would apply to everyone, not just males.

What does Well-regulated mean? I like to pick out a contemporary piece that contemplated the term and character of such a militia. Alexander Hamilton in Federalist paper 29

The 2nd amendment clearly *references* a right. It doesn't *define* one in full detail.

The founding fathers considered there to be a natural right to keep and bear arms, which was so commonly understood as to not need explication (and in fact was a thoroughly established feature of the English common law on which American law was based) and should not be infringed by the US federal government.

This was not an absolute and unlimited right for all persons to own whatever arms they wanted and bear them at any time and place of their choosing. In their time, there were laws against things like "affrighting" -- causing a disruption of the peace by carrying an unusual and alarming weapon, according to the standards of the community.

The right to keep and bear arms was a complex notion. It didn't mean convicted felons or officially recognized madmen could have guns, or that you must be permitted to carry a loaded pistol in town. It did mean a common citizen with no criminal record must be permitted to store firearms and ammunition in his farmhouse, and to carry them for defense when travelling.
