You're overthinking this.
It's pretty simple. You need a plan of attack that cannot be spread beyond intended target areas. This rules out any airborne/foodborne pathogen.
You want the land to still be useful when they are gone, this rules out chemical (VX, Sarin) and Nuclear.
Conventional attacks drive survivors underground, forcing a protracted campaign which may never be 100% successful.

Write a retro virus that specifically targets the world's largest current plague: AIDS.
It rewrites the RNA of the AIDS virus and causes it to present with hemorrhagic fever and cause death via exsanguination instead of destroying white cells and crippling marrow.
The retro virus ONLY interacts with AIDS and HIV. This is easy as both viruses have very unique RNA sequences. Since this is a tailor made retrovirus, you can also create a perfect loophole to deactivate/destroy it and introduce that into the populace via the water tables.
