It probably works because fluoride kills every microrganism it comes into contact with. In acidic oral environments caused by the bacteria, the tooth dissolves, theoretically releasing that stored fluoride and killing the bacteria.
Or you could just brush your teeth with baking soda. I had some dentists tell me I needed to have a tooth filled as I had a cavity. I told them I'd wait for it to heal on its own and they laughed as if that were impossible. So I starting brushing better and sure enough it healed. No fluoride poison necessary. And they still compliment me on my teeth every time I go in

It can be avoided by keeping the clean of sugars of course, but likewise by brushing with baking soda you brush the sugars off the teeth and create a very alkaline environment in the mouth.
I usually brush with nothing but a pinch of baking soda. It keeps my mouth 100% clean all night. No fluoride necessary and its so cheap. As a bonus it whitens the teeth, and don't worry, its softer than tooth.

The bacteria that live in your mouth will create an acidic environment on their own (sugars help but are not necessary). Note that all of those are bacteria that live in a low-oxygen environment - brushing itself throw them in middle of your mouth, exposing them to oxygen and killing most of them.
Of course brushing without some sort of chemical agent to reduce acidity (like baking soda, as you say) won't stop your enamel from dissolving. If you are really strict with your brushing regime (like after every meal, before sleeping and such) suppose you don't need a source of fluoride as your enamel shouldn't dissolve too much, but if you only do morning + evening brushing and don't watch what you eat I would recommend fluoridated paste at least at the evening brushing (the evening one is much more important because more bacteria develop in your mouth as you sleep because less air passes through meaning lesser oxygen concentration)
