In some cases, it's due to meddling from foreign countries. Vast swathes of Africa could be Eastern Europe-tier by now if it weren't for the West. And let's not forget what meddling in the Middle East has done more recently in places like Libya, Syria, and Iraq.
For the rest, it's mainly due to corruption. But even in those cases, the leaders or regimes are often propped up by outsiders (see: Haiti, North Korea, etc.).
