Depends on the person. In most cases it's most likely a legit mental disorder. Being gay is technically a disorder, but it's one that really doesn't completely hinder your life experience by itself so I don't really care about it. Not being able to come to terms with your gender identity however WILL fuck you up. There's a reason that these people have such high suicide numbers. It's like those people who legit see themselves as animals. If you do not identify as yourself, as you are, it WILL fuck you up in the long wrong unless you get help. People for some reason just think pandering to them will work even though it really won't.
