The primary cause is simply that Europe opened its borders. When the borders are open, the undesirables are going to flood in.
More are trying to flood in than usual due to NATO blowing up the Middle East and Northern Africa using terrorist proxies and direct bombing. But no matter how much you worsen their homes, not a single one would be immigrating if treacherous governments were not welcoming it.

If you have to assign blame to one person, then it would be Merkel. If you're willing to spread it around to a handful of people, then it's Merkel and her cronies in the EU. If you are willing to spread it around a lot then you could blame the entire intellectual elite in Europe in addition to the EU's leadership for even permitting this to happen. If you want all encompassing blame for the entire situation then blame the USA for destabilizing and actively worsening the political climate of the Middle East for the last 13 years.
