This is a rather hard to define point, you are absolutely right. However, if I had to draw at least a minor boundary just for the sake of example, I would say that you are intentionally utilizing a foreign culture as a means of inflicting injury on one or more persons.
For instance, mocking a culture for the sake of comedy can actually be beneficial, because they intention there is to deconstruct stereotypes. This is because if you can take a facet of a stereotype and turn it into a joke that people can laugh at, then you steal away the ability of the stereotype to inflict injury. Essentially, with comedy you have made a caricature of the stereotype itself, not the culture, although on the surface it may appear life you are attacking the culture itself.
This example can be contrasted with one that is clearly meant to cause harm and inflict injury. For example, imagine if someone were to use satire to reinforce a stereotype, as opposed to attempting to weaken it. What they are doing there is attempting to undermine the dignity of a people and their heritage, rather than attempting to draw elements out of it to use in a creative way.
No matter how you slice it, though, there will always be people who perpetually imagine slights against them because the have been trained to fabricate stories of victimization and to see the innocent actions of others as always being malevolent, even when they aren't. They all effectively have Paranoid Personality Disorder.
Just remember that perception is not reality, and their stories are usually made up bullshit. Stand your ground and punch back twice as hard with the truth.
Leave them no safe harbor. Question everything. Demand evidence. Always remain skeptical, and demand this of others. Brutally call people out on their bullshit and hypocrisy.

I would say that Halloween costumes are a gray area. It would depend on how they are worn, what is portrayed, how you behave while wearing it, and several other considerations. Somehow I cannot get behind the idea that it is racist for a White guy to wear a suit of samurai armor for Halloween. I mean, yes, it is partially a cultural thing, but it is just a suit of armor, even if only a cheap costume facsimile.
Now if someone were to dress up in as a race itself, or something strongly tied to that, and then went about spending the evening acting out the worst stereotypes of the culture they are portraying, their actions should probably be condemned. It really all just depends on the context, the intent, and what is done with it.

I agree with you, but I do see where people are coming from in complaining about this. Bushido was something held in the highest reverence and now some weeb is comically wearing a costume that represents it. It's making a joke out of something sacred. It's almost like someone dressing in the attire of a priest as a costume. Catholics would be very upset about that and most people would say rightfully so.
Now, no one follows bushido and isn't seen as a sacred thing so I would say it's fine to dress up as a samurai. Nevertheless, it's something to keep in mind.
