If you mean in the first world capitalist or centrist countries, you're right that almost nobody is dying directly of malnourishment. However, those areas rely to a large degree on exploitation of labor and resources in developing regions, many of which have some of the worst issues with hunger and disease on the planet.
For example, Singapore has essentially no hunger or poverty within its borders, but it relies very heavily on foreign-born workers and imports from developing regions. These imports and workers often come from areas like India, Bahrain, or Southeast Asia, because those regions are poor and imports from said areas are quite cheap.

when we engage in trade with another civilization, both parties benefit. it's the cornerstone of economics. we dont pay them much because they have low capital stock, but increased trade leads to increased affluence in poor countries which leads to investment.
exploitation occurs, but it's almost exclusively by local governments that bar free trade

I never said that it's a good idea to sever trade with poor countries. I was saying that bosses of laborers in poor countries should stop suppressing union activity and should raise wages based on labor input. Also, either the government or economic bosses should more widely provide services like healthcare and housing, whenever they can pay for it. Also, governments in developed countries should stop propping up corporate elites, and sever aid to pro-foreign-corporation despots like Nguema and Karimov.
