It certainly didn't change in 4 years.
Nearly everyone speaks english. As in, can bumble their way through a conversation, barely. A good amount of people do speak proper english, and most of them will be in the cities.
The further you stray from the city, the more necessary english gets. There's also still a generation alive in the east who was taught russian in school.
But so long as you speak english, you'll never be accepted. You'll always be the foreigner and outsider.

That isn't to say it's always worth it. If you consume a lot of media originally in one language, you should learn it, both for the above point and the culture behind it. If it's one-offs, not worth it. Accept the watered down version and move on.

I'm foreign and work for a company with a bunch of foreigners. I can tell you that you can get by with just English fine but it's not fun at all if you ask me. Most Germans are able to communicate in English well, especially the younger ones, but it's just not the default language and you feel it. Anytime someone starts talking to you it's in German so if you don't understand you feel dumb and like an outsider.

That said, some of my coworkers have different experience; like the ones who don't look German (this guy in particular is Indian) don't experience that people start talking to them in German randomly. Depends on how good your german is. If you can have fluent conversations it's better to speak german. If it's hard for you english is fine but many lower educated people in service jobs have problems understanding english.
