A year ago I would have agreed with everything you posted because there was plenty of evidence to back it up. But for reasons not yet understood, birthrates in Africa have started going back up in areas where all of that has already been accomplished. Now we have areas with low child mortality and rising fertility despite good access to medical care (relative for African standards) and family planning. It's a pretty worrying new trend and hopefully will turn out to be few year long fluke. If not, things are going to get really bad really quickly.
