Poor diet, lack of general exercise, laziness.

Eating generally isn't something people do to enjoy here, it's just an event to feed the body. Americans do not want to spend the time preparing decent nutritious foods because they would rather be doing something else.

I think there are several factors.
1) The massive shift to processed food diet. Not sure why they were so quick to accept it, though, or why it took more time for everyone else.
2) Race diversity, with some races having different cultures (or lack of) and massive difference in income (poorer people are fatter in rich countries). Much of USA's fatness is because of blacks and hispanics.
3) Much of the country is massively far away from the beach. As stupid as it sounds, I think never showing your body in public makes it easier to indulge in shitty diets.

There are people here that honestly can't remember the last time they drank a glass of plain old water because the drink so much coffee, sweet tea, milk, soda, and other sugary crap.
Our vegetables are too often drowned in cheese or ranch dressing. There are fried versions of just about every food known to man. Even my small town of under 8,000 people has 4 different pizza parlors, 5 fast food joints, and 2 Chinese take out places.
Everyone has cars and will drive to places less than a mile away. Most of jobs require little to no physical exertion.
