I was very fit a while ago then i got crohns and lost all my muscle and have spent the years since waving about with vitamin deficiencies and vitamin overdoses. So most days my joints hurt and i do get out of breath.

It doesn't mean they're not fit, it just means they don't do anything to improve their stamina, such as working out.
Reality is, in a modern society working out only gains you attractiveness (which some people don't care, and some work against - for example muh fat shaming).
Of course, we'd all be happier and healthier if we actually took care of ourselves, but people don't bother because your survival doesn't directly depend on it.
