who gives a shit? It's mostly a religious thing unless a doctor notices you have a urethral condition, so in general it shouldn't be done. Japanese mostly don't start out under the cultural vision of cutting off penis bits, so it's not a standard thing.

I looked it up because you made me curious and the pages I found about it said that if you want it done in japan you're going to have a pretty tough time finding a surgeon who is willing to do it, so I guess it's not illegal but there's no demand among patients and no doctors are willing to do it because they see it as crazy and barbaric and they might lose their licenses if they start performing abnormal and unnecessary surgeries.

Japan's healthcare system is socialized and child healthcare is free, so the government specifies the fees for medical procedures.
Cosmetic surgery isn't covered so you can get rich doing that but I doubt parents would take a kid to a plastic surgeon and ask to have his dick streamlined
