Much better to cut and avoid putting unnecessary strain on the baby's head if there is too little room. My wife just had our baby on December, so we prepared for this by doing some research on the topic. So here are my two cents:
The doctors put the mothers through a lot of unnecessary and annoying screenings. I have heard cases of women that prefer to go outside to the parking lot and have the baby there rather than the hospital. And I completely understand
Our experience was basically me and an assistant that we hired for the delivery to fight against the fucking doctors. These guys are basically just waiting for the first signal of weakness, for the first excuse to bring their fucking instruments, drugs, and all their bullshit. If you give them an inch they will try to fight for a mile unless you stand against them. It got to the point that we had to tell plainly tell them to fuck off.
We told them, we dont want your drugs, we dont want your procedures, we dont want any of your shit, we just want to have this baby naturally, and they HATE THAT. If you ask doctors around most of them will tell you that is extremely rare for women to deliver babies completely unassisted in the hospital.

You get to pick your ob/gyn and hospital. While that particular ob/gyn may not be awake at the time your baby decides to pop out, they generally all have relationships and recommendations. They will go over who will be delivering, or a back up at least.
My wife reviewed a couple of different groups and decided on one. The doctor was super cool and competent.. even was able to pick up without saying a word that I had absolutely no interest in cutting the cord or looking at the placenta.
I have only been present, as an adult, at one birth, but everything was pretty controlled, explained, and relaxed.

Nurses are the ones who are monitoring the patient 24/7 and know their patterns and behaviors the best, but have to call the doctor to get permission for every little thing before they do it.
I live near a big teaching hospital, and all the nurses there have a big meeting right before all the new-graduate doctors come in to work. Basically, the nurses take all the new doctors under their wing and tell them how to do their fucking job, even though the doctor is making 10x more than they are. It's all about doing the research about your doctor.
I just gave birth last Nov., 2015, and I had a wonderful doctor, and a great team working with me. I had a very complicated pregnancy. To the point I needed 2 supportive surgeries at 20 weeks, and the one other patient my Obgyn who had a similar case, lost the baby.
But we soldiered on, and at 39 wks, 4 days my water broke. Most of the time for first time mothers your water doesn't break before labor. So I was rushed to the hospital, and my labor lasted for 24 hours. My cervix would not dilate, even after 24 hours of drugs, and too much time had passed and the risk of infection was too high so I was told I could continue natural labor and wait for possibly up to 2 more days or I could get a highly recommended c section. Which I did, my daughter was born about 5 mins after they cut me open and it went beautifully. I was walking on my own within 12 hours of the surgery and my daughter was born perfectly healthy.
The signs I got that were good about my obgyn is he visited me everyday I was in the hospital at the 5 week mark and all the staff who knew my obgyn had nothing but great things to say about him. My pediatrician had his son born to my obgyn about 10 years prior, so he came highly recommended.
For someone going through their first pregnancy, I cannot stress enough RESEARCH! So important to know who is doing your procedures and what their reputations are.
Also, half these women are just bitching about normal things. Yes some severe accidents happen, but complaining about a doctor using their hands to make sure the baby comes out correctly is hardly one of them.
I almost died twice to have my baby, and she was worth it. These women making the signs are batshit and just need something to bitch about. I feel sorry for the mothers who lose their children due to legitimate malpractice though. I can't even imagine their pain.
