The former is largely due to his grandstanding and smug attitude, while the latter is largely due to his propensity to race bait, which he did with Trayvon Martin, Michael Brown, and Ahmed Mohammed, way before the facts actually came out.
Those are the big 5 that I can think of off the top of my head. There are plenty more which fall more under the category of things he DIDN'T do. I really can't think of a single meaningful achievement other than maybe Osama Bin Laden.
