It's not unimaginable that transgenderism in males (male body, female brain) is caused by some error during fetal development that prevents the brain from being masculinized. anyway, it is medically agreed upon, that it is a mental illness. it can be diagnosed and treated, either by getting the patient to accept themselves the way they are or in extreme cases by adapting the body to make them feel better. it's basically an extreme case of body dismorphia.
body and mind are so intertwined that it's hard to tell how much of it is mental and how much physical. even classic mental disorders (like depression) almost always have a physical component to them (like hormone imbalances) - hence why there are meds for psychological disorders. and in the end any mental disorder will be physical in some way, since our mind results from the way our physical neurons are connected. we just don't understand enough about our brains and so we call it "mental".
i think it depends on the individual. there'll certainly be some with sex hormone imbalances at some stage of their development which results in a different mental development. there'll be others where there's seemingly no physical cause.
