They're talking about the 21 million people who were forced to get healthcare through Obamacare. The idea is individuals who want healthcare will be able to get better and cheaper healthcare while those who don't really want it, aren't forced to have it.

Yes, you can read the report yourself. It's not hard to imagine this being true. Per capita, US healthcare expenses were higher than every other nation in the world even though it didn't have universal healthcare. It's possible to lower healthcare costs significantly and bring it to everyone. If you go to a doctor's office in the US you can see the issues. They can charge whatever they want for doing very little. Obesity is also an issue affecting healthcare spending but Americans like being fat, specially in the South.
