coke is just used to refer to cola drinks. if they want something besides basic cola, they ask for it by name, like Dr. Pepper, or Sprite, or Mountain Dew.
i think it's more prevalent in the deeper south because coke was invented and is headquartered in atlanta.
for a lot of restaurants and diners in the south before 1960, coke was all you could get, so that's what stuck. to this day, 90% of restaurants either serve coke or pepsi, not both.
