The only way to truly enforce this is to either impose a wage based import tariff on H-1B labour or create competitive minimum wages for H-1B visas so they do not compete wages down. Any other solution would create a bureaucratic mess because the difference between 'skilled' and 'unskilled' is unspecified and subjective.
