Actually, here in Germany the way you get around the single payer lines is by simply living in Germany where there are hundreds of insurance companies and pay privately.
Germany is sort of America's healthcare insurance system gone completely fucking capitalist with a few Obamacare elements. We don't have a "pre-existing conditions" carve-out for private insurance, so for instance I was rejected by 3 private insurance companies and only accepted by government insurance after 12 months and a penalty payment when I moved back from the US.
