My guess is this was an accident while reloading.
Not reloading like putting a fresh magazine into your weapon, but reloading as in cleaning and resizing spent brass, and seating a new primer, powder, and bullet.
This guy was probably reloading a bunch of ammo, and pulled the lever on his reloading press without moving his other hand out of the way first.
This is why you have a casing with a live primer but no bullet.
