Firstly, even though it is only a small majority of them that are extremists, it's still a massive number. ISIS, Boko Haram, the Taliban and all the other organisations that plague the middle east and north africa rank into the hundreds of thousands (Mid to high estimates >200,000), which is bigger than the larger European armies.
Secondly, save Indonesia, human rights offences and quality of life in Islamic countries is pretty damn appalling. These include
