Black jack was written in the 60's or 70's when racism was a thing.
The skin on black jack's face is black because he needed a skin graft and his black friend gave him his skin.

I don't necessarily think so, it was mainly tezuka's idealized version of himself since he was a trained doctor but decided to draw manga instead. He's tezuka's ideal of a doctor antihero character - he can accomplish any kind of impossible surgery, but he charges people hundreds of thousands of dollars. However, he waives the fee if he decides that the patient is an ethical person, which happens in most of the stories. He still robs them blind if they're scumbags though.
Kind of like a Robin Hood who robs the rich by offering to make them healthy rather than hurt them.
