The audience was mostly teenage girls. Their were probably <50 adult men in the audience most of whom would have been there with their daughters.

Well there in my post you see people were fleeing and finding exits - I must note the fella claimed he fled onto stage and found exit WHEN THEY WERE RELOADING...
So perhaps there was plenty of opportunity - the crowd scattering away as fire ensues though leaves distance when reloading starts... also if they covered each other and alternate timed reloads...though the fleeing fella did nott give that impression but the opposite..
lots of noise and screaming affords opportunity to attack unnoticed... in any case it appears the venue was full I just don't see the count yet .. and police failed to enter for at least 2 full hours after the killing started - not sure I guess the shitstains were on break for a couple hours - the reports claim no negotiations or talking... so whatever maybe they were raping some hot french asses ... for a couple hours

I probably would drop to the ground and stay there willingly/not be able to get up if I was, say, shot in the shoulder.
