A band wants to use one of my photos for their album cover. Does anyone have a good licensing agreement template or advice regarding this situation?

How much money is going to be involved? Is it a band that only makes $300 a year playing local shows or somebody signed to a major label?

Charge annually and for every re-issue. Don't forget t-shirts, advertisements/promo.

Art can be worth a shit load of money if you play it right.

Buy the Graphic Artists Guild book guide to etchics and pricing. It will help you a bit. Photography is really kinda no different.
