if you operate the engine at a very low RPM with high loads you will increase cylinder head pressure and increase chances of detonation. you will also break the film on the oil which will cause wear on the bearings.
Its like when you try to start out in a bicycles highest gear. It takes alot of effort to accelerate.
A 4 cylinder engine doesnt make alot of torque because a power stroke lasts for about 180 degrees of crankshaft travel so you need to keep rpms high for it to use momentum combined with the pwoer stroke to keep going, this is why you can cruise in a V8 at very low RPM's compared to 4cylinders because v8s have double the cylinder and each cylinders power stroke is effective for 90 degrees of crank travel.
