It's a cost-benefit decision made by accountants and lawyers. Tesla is a young company that is highly overvalued and a class action lawsuit for seatbelts would probably kill the company. The benefits of recall outweighed the costs versus the cost of the lawsuit

shit like this happens, its a supplier issue, i'm assuming Tesla does Out of Box checks for all their components and randomly tests them, relying of course on the supplier and that they did the proper stress testing on the assemblies before releasing them to the customer (Tesla).
