I like the safety/suicide/extension levers on my '80s bike, but they are a little chintzy and mushy feeling. Any (preferably non-aero) "modern" or good retro solutions that allow braking from all bar positions?

Not really. Do you keep a foot on the brake when driving? Riding on the tops gives you much less leverage on the bars so you don't really want to be there if you have to finesse control of the bike anyway. It's better for climbing or just putzing along when you're certain of everything
